I acknowledge the inherent risks of martial arts training at The Academy, including strenuous activity and physical contact, which may result in injuries from bruises to paralysis or death. I accept these risks and agree to assume full financial responsibility for any injury or death to myself or others. I agree not to sue and will submit any claims to binding arbitration (an independent third party outside the court of law).(Required)

I have no health problems that prevent participation, such as asthma, diabetes, epilepsy, or heart issues. I will inform the instructor of any condition that may endanger myself or others. I confirm I have the necessary skill and fitness to participate safely. I agree to follow all safety rules and wear required safety equipment as deemed appropriate by The Academy. In case of emergency, I authorize necessary medical care and assume all costs involved.(Required)

I hereby release The Academy, owners of the premises, its management, employees, volunteers, agents, and servants from any and all claims, liability, demands, or rights of action in the event of ordinary negligence that may occur during participation in warm-ups, equipment use, instruction, supervision, and use of the facility. This agreement shall be binding upon me, my spouse, heirs, representatives, executors, assignees, or transferees for the extent of my membership.(Required)

I agree to hold harmless and defend The Academy and its owners, management, employees, volunteers, agents, and servants against any claims arising from injury, death, or property damage caused by my negligent or intentional actions during activities at The Academy. I indemnify them against claims made by co-participants, rescuers, or others if any participant is injured. I agree to reimburse them for any attorney's fees, court costs, and legal expenses incurred to enforce this agreement.(Required)

I hereby give permission to Hawaii Jiu Jitsu Academy LLC to use images of myself or my child, captured through video, photo, or digital camera while participating in academy services and on the premises. These images may be used solely for promotional materials, publications, advertising, and web content. I waive any rights to compensation or ownership regarding videos or images.(Required)

I have read and understand this agreement. I give up significant legal rights, including the right to seek compensation for injuries from inherent risks or The Academy's ordinary negligence. My participation is voluntary. I release Hawaii Jiu Jitsu Academy from all liability for any injury or loss due to inherent risks or their ordinary negligence. If any part is invalid, the rest remains effective. This agreement supersedes all prior agreements.(Required)
