I acknowledge and understand the risks involved in martial arts training, including injury or death. I accept full responsibility for my participation, attest to no health issues that could prevent my participation, and release Hawaii Jiu Jitsu Academy, its owners, employees, and representatives from liability in the event of ordinary negligence. I agree to indemnify and defend The Academy against claims arising from my actions and authorize emergency medical care if needed.

I hereby give permission to Hawaii Jiu Jitsu Academy LLC to use images of myself or my child, captured through video, photo, or digital camera while participating in academy services and on the premises. These images may be used solely for promotional materials, publications, advertising, and web content. I waive any rights to compensation or ownership regarding these images.
